JOB SUMMARY:

The Team Manager (TM) works in collaboration with the Director Clinical Operations (DCO) as well as the Interdisciplinary Group (IDG) to ensure that care and services provided by the team are delivered in a fiscally responsible manner while maintaining strict compliance with all federal, state, and local rules and regulations. The TM, in collaboration with the DCO and IDG, will assist in ensuring that appropriate and quality services are provided through the continuum of care.


REQUIREMENTS:

    • LPN or two (2) years of experience in a healthcare setting
    • Experience using an electronic medical record (EMR) system
    • Familiarity with healthcare regulations
    • Must have and maintain transportation to be used to and from your assigned place of business
