Voyager Home Healthcare (https://voyagerhomehealthcare.com/) provides in-home Nursing, Certified Nursing Assistants, Physical Therapists, Occupational Therapists, and Personal Care Worker services throughout the state of Colorado. Additionally, we have two CNA schools (https://www.getcna.co/) which are unique in that they allow students to take their classes in an online format while enabling them to attend in-person clinical and practice labs. We recently opened office locations in Denver and Pueblo and are currently in the process of expanding our home healthcare services and CNA schools to the states of Florida and Arizona.

Duties

• Perform thorough evaluations of the patient's functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
• Make home visits to clients in the community, as well as Senior Living, Assisted Living, and Independent Living facilities.
• Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans to achieve favorable outcomes.
• Evaluate the home environment for hazards or barriers to more independent living.
• Identify equipment needs and environmental adaptations resulting in enhanced outcomes for clients.
• Continually assess and revise the occupational therapy care plan, and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Other duties as directed by the physician, Clinical Manager, Director of Nursing, or personal care plan.

(Some weekends may be required if there is an emergency visit)

Pay: $100,000/year
