Southeast Colorado Hospital District (SECHD) has been serving the people and communities of rural Baca County for over forty years. The District was established in 1966 as a not-for-profit hospital and officially opened its doors in 1969 with 23 acute care beds. Since that time the District has followed its vision "to provide a comprehensive healthcare system to our isolated rural service area" through an active pursuit of enhanced health care services. Some key services added in fulfillment of this vision include the addition of a 40 bed intermediate care Long Term Care Center, establishment of a Home Health Agency and Hospice , and the building of a 16 bed secured Alzheimer's Care Unit. SECHD's vision continued when the Specialty Physicians Clinic program was developed, and most recently in 2007 when the new Physicians Medical Clinic facility was completed.

POSITION SUMMARY:

Assists in the planning, directing, and evaluation of programs related to resident care and to provide consistency in nursing functioning in conjunction with of the Assistant Director of Nursing.

ESSENTIAL JOB DUTIES AND RESPONSIBLITIES

· Reviews and assists in keeping nursing care plans current.

· Assists with mandated assessment forms and evaluations.

· Assists with development of nursing staff and scheduling.

· Collaborates with ADON to achieve desired outcomes for residents, families, and nursing staff.

· Completes MDS’s and matrix.

· Reviews with residents and families IDCP at Interdisciplinary Care Conferences.

· Reports QAPI’s. Completes weekly timecards for LTCC.

· Coordinates and participates in monthly medication review.

· Completes assignment list for monthly nursing summaries at LTCC.

· Coordinates and prepares discharge planning with physicians, midlevel providers, social services, and home health.

· Attends quarterly P&T and antibiotic stewardship meetings.

Required Education and Experience

· Current RN license in good standing in the State of Colorado

· Basic Life Support (BLS) certificate.
