Systems Navigator/Mental Health Practitioner - Behavioral Health Homes
Woodland Centers is currently seeking a full-time Mental Health Practitioner to function as a Behavioral Health Home Systems Navigator within a multidisciplinary team working with youth and adults. This is a Monday-Friday position based in our Willmar office, though some travel within our multi-county catchment area is required. In this position, you will be working in our Behavioral Health Home (BHH) and you will work closely with a network of mental health, primary care, hospital, and community providers to address the client’s goals for physical health, mental health, substance abuse, and wellness.
Job Duties:
- Assist in stabilizing client’s mental health, chemical health, social health, and medical needs.
- Provide ongoing coaching, support, motivation, encouragement, and positive feedback.
- Interact with clients on the phone or in person on a regular basis to build trusting relationships and assist the client with identifying barriers to accessing care.
- Meet with clients before health care appointments to review client concerns and/or after appointments to review provider instructions.
- Assist clients in accessing resources in order to follow provider instructions.
***MUST MEET MENTAL HEALTH PRACTITIONER REQUIREMENTS***
“Mental Health Practitioner” as defined by MN Statute 245.462 is defined as a person providing services to adults with mental illness or children with emotional disturbance who is qualified in at least one of the following ways:
1. Through relevant coursework:
a) Has completed at least 30 semester hours or 45 quarter hours in behavioral sciences or related fields AND:
i. Has at least 2,000 hours of experience providing services to individuals with
a mental illness or a substance use disorder, a traumatic brain injury or a developmental disability, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to adults or children; OR
ii. Is fluent in the non-English language of the ethnic group to which at least 50% of the individual's clients belong, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to a client; OR
iii. Is working in a day treatment program under section 256B.0671, subdivision 3, or 256B.0943; OR
iv. Has completed a practicum or internship that required direct interaction with adult or children served AND is focused on behavioral sciences or related fields, or is in the process of completing a practicum or internship as part of a formal undergraduate or graduate training program in social work, psychology, or counseling.
2. Through relevant work experience:
a) Has at least 4,000 hours of experience in the delivery of services to individuals with a mental illness or a substance use disorder, or a traumatic brain injury or a developmental disability, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to clients; OR
b) Receives treatment supervision at least once per week until meeting the requirement in clause (1) of 4,000 hours of experience and has at least 2,000 hours of experience providing services to individuals with a mental illness or a substance use disorder, or a traumatic brain injury or a developmental disability, and completes the additional training described in section 245I.05, subdivision 3, paragraph (c), before providing direct contact services to clients.
3. An individual is qualified as a Mental Health Practitioner if the individual has a master's or other graduate degree in behavioral sciences or related fields.
