Remote LCSW-R in New York State and other states

The ideal candidate will be licensed as an LCSW-R in New York and 1 or 2 other states.

The Company was formed to offer nursing homes, assisted living and post-acute care facilities a no cost option for enhancing continuity of care for patients. Tapestry provides the technology, personnel (internal medicine and specialty care providers, administrative assistant(s) training, and online lab and medication ordering – all at no additional capital cost.

Position Overview: Seeking an LCSW-R for this position to be part of a multi-disciplinary care team accountable for providing clinical social work services to support Tapestry’s geriatric behavioral health. Tapestry provides most of its behavioral health via TeleMedicine.

Job requirements include:

• Consideration of developmental, social and/or cultural, substance abuse, psychological, environmental and medical issues
• Develops treatment plans including measurable treatment objectives
• Provides clinical treatment services in form of triage, crisis intervention, individual, group and/or family therapy, screening and planning
• Advocates and develops networks of social and clinical services to assist clients in meeting identified needs
• Monitors and evaluates effectiveness of treatment plans, documents treatment in clinical records
• Completes reports in accordance with agency and accrediting bodies
• Consults and collaborates with other behavioral health providers to improve delivery of services to clients
• Maintains social work treatment and standards as required
• Performs related duties as required

Knowledge, Skill and Ability

Knowledge of social work methodology, casework, group work and community mobilization; knowledge of family and interpersonal relationship dynamics; knowledge of values, sanctions, purposes and ethics of professional social work; knowledge of social, cultural, economic, medical, psychological and legal issues which influence attitudes and behaviors of clients and families; knowledge of mental illnesses and approaches to treatment; considerable interpersonal skills; considerable oral and written communication skills; ability to devise and implement a treatment plan with measurable goals that address client needs; ability to independently apply current psychiatric treatment modalities to address client needs.

Qualifications Required:

• LCSW-R
• Licensure as a clinical social worker in the States of New York and other states. It will be expected and the company will assist with obtaining licenses in additional states.
• Experience as an Alcohol and Drug Counselor (certification preferred)

Salary and benefits: $50 - $70 per hour. Pay is based on several factors including but not limited to education, work experience, certifications, work location, etc. In addition, for full-time employees, we provide a comprehensive benefits package, 4 weeks of PTO, 9 company holidays, CME time off, annual CME allowance, and malpractice insurance.
