POSITION TITLE: CHARGE NURSE

SUPERVISES: NURSING ASSISTANTS, BED MAKERS, HOUSEKEEPING, DIETARY AIDES

STATUS: NON-EXEMPT

RELATIONSHIPS WITH: ALL STAFF IN NURSING HOME, HOSPITAL, AND CLINICS
JOB SUMMARY * Oversees resident cares; administering medications and treatments.

  • Maintains accountability with staff for residents daily cares.
  • Communicates changes in resident health status to MD/APRN and resident and/or family.

EXPERIENCE & SKILLS:

Must be able to communicate verbally and in writing.

EDUCATION & TRAINING:

  • Must have current CPR certification.
  • Prefer experience with intravenous therapy.

EDUCATION AND QUALIFICATIONS (preferred):

Required Licensure/Registration/Certification: Must have current license in State of Minnesota.

SPECIAL SKILLS:

  • Must be able to read, write, speak, and understand the English language.
  • Must be able to function independently, be flexible-time wise, and have the ability to work effectively as a team member when the opportunity arises.

POSITION SUMMARY

RESPONSIBILITIES:

  • Observe and report symptoms, condition of residents and/or significant changes to Physician, APRN and DON, appropriate ancillary professional staff, and responsible party.
  • Administer medications and record in eMAR in a timely manner. Unless the resident has been assessed to self- administer medications, the nurse must observe the resident as they take their medications.
  • Watch for refill medications 1-3 days before expiration date and order from drugstore of resident’s choice.
  • Chart information reflecting resident/patient’s condition.
  • Make rounds with the Practitioners.
  • Assist Physicians/Practitioners in treatments and resident/patent examinations.
  • Bathe and do resident care if needed.
  • Teach and supervise Nursing Assistants.
  • Maintain aseptic technique to maintain infection control.
  • Record phone and verbal orders from physicians/APRN.
  • Answer residents call lights.
  • Supply information of residents condition at nurses discretion to family or responsible party, if consent allows.
  • Supervise and discipline visitors.
  • Admit, transfer and discharge residents per physician orders.
  • May allow Leave of Absence to residents at nurses discretion of residents condition.
  • Supervise dietary needs of the resident.
  • Final preparation of deceased residents.
  • Contact family and minister on record when death seems inevitable.
  • Maintain responsibility for an adequate quantity of supplies and equipment.
  • Assign work schedule for Nursing Assistants.
  • Maintain current knowledge of the fire and disaster plan for evacuation of residents from their room.
  • Uphold responsibility for supervision of meals from dietary kitchen and the sanitation of resident rooms.
  • Maintain responsibility for supervision of medical charts and laptops, update eMAR, and treatment orders.
  • Must have knowledge of the Matrix computer system , Point of Care Charting and eMAR
  • Carry out treatments ordered by the practitioners and care outlined on nursing care plan.
  • Update nursing care plan according to resident’s abilities.
  • Assist in orientation of peers.
  • Give fleet, oil retention or any other medicated enemas.
  • Give all vaginal douches.
  • Maintain responsibility for intravenous therapy. LPN may hang IV medications that do not contain medication, change tubing and perform saline flushes. RNs may start IVs , flush IVs and change IV tubing and hang IV fluids that contain medication or give medications IV push as ordered .
  • Assist staff in completing resident and employee incident/accident reports. Inform DON immediately.
  • Attend monthly staff meetings or read minutes to improve job knowledge.
  • For RN and/or LPN: Engage in performance appraisal and competency evaluation on a regular basis, identifying areas of strength as well as areas for development for self and Nursing Assistants.
  • Report any defective equipment or environmental hazards to appropriate personnel immediately (i.e.: maintenance department).
  • Wipe up spills and removes other hazards immediately that could cause a fall or injury.
  • Perform admission assessments of all residents.
  • Follow established policies and procedures within the nursing department.
  • Maintain awareness of Federal and State regulations as they pertain to nursing services.
  • Perform any other job related duties as directed by DON or Unit Coordinators.
  • Assist with hourly rounding.
  • Maintain order and cleanliness of medication carts and medication room.
  • Removes all outdated medications and supplies form medication room and medication cart.
  • Participate in Quality Improvement projects and audits.

  • Must be able to handle difficult and emotionally upsetting circumstances with dignity, which involves staff members, clientele and/or family members.
  • Must be able to cope with the mental and emotional stress of illness and death.

Prolonged walking and standing with frequent turning, bending, stretching while using proper lifting techniques.

  • Participate in Disaster Plan of Action.
  • Participate in Universal Precautions for AIDS, Hepatitis and Tuberculosis.
  • Follow facility established protocol for Isolation and Blood-borne Pathogens.
  • Respond to Code Blue’s.
  • Complete yearly assigned in-services

CONFIDENTIALITY:

Frequent contact with patients and residents and medical information in the normal performance of duties. Confidentiality is essential. Disclosure may affect internal and external relationships.

Nursing Home RN – Full-time position. MN licensure required. Must work every other weekend and additional compensation for shifts and weekends. $4,500 Sign-on bonus.
