Care Manager Supervisor

About the Agency: MedAssist empowers patients facing medical challenges to thrive by addressing their needs with comprehensive access to services through care management. Our Agency is expanding and we are looking to add some great people to our team!

Position Overview: As a Care Manager Supervisor, you will have oversight of a team of Care Managers who provide services to patients who are returning home from nursing homes or rehabilitation facilities. This job entails ongoing supervision, communication & feedback relative to patient care, staff guidance & support, quality assurance, and ensuring key performance metrics are being met. The Care Manager Supervisor sets the tone for the team in assisting patients with enhancing their health & overall well being by building lasting connections with them. This position reports to the Chief Operating Officer.

General Responsibilities (including, but not limited to):

• Review, guide, and support the Care Management team with researching & coordinating various services such as physical therapy, medical supplies, transportation, etc., in alignment with patients’ needs.

• Oversee the Care Management team relative to patients’ progress, needs, and continuity of services to ensure quality of care.

• Review Care Managers’ patient notes in the electronic health record for accuracy, timeliness.

• Follow-up with Care Managers promptly to ensure adequate service delivery is met.

• Perform other duties as assigned.

• Management experience (highly preferred).
