Top National Healthcare company is hiring a LSW, LCSW, LMSW - Case Manager in the Leesburg, VA area!

***This is a Hybrid role - Will be required to do Field based work and work at home when not in the field.***

Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues

SW Case Manager is responsible for telephonically and/or face to face assessing, planning, implementing and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies.

• Determines medical necessity/ appropriateness
• Facilitates optimal outcomes
• Identifies and follow through with continuous quality/ compliance opportunities .
• May also include identification of aberrance's and initiation of corrective action
• Educates/ empowers customers to ensure compliance, satisfaction and promote patient advocacy
• Optimize total costs
• Implementation and evaluation of policy based on usage and program directives
• Educate/empower colleagues at all levels to enable decision making at most appropriate level

Requirements:

• Must be fully vaccinated for Covid-19
• Must hold Unencumbered and current LSW, LCSW, LMSW in Virginia
• Minimum of a Master's degree in Behavioral/Mental Health or related field
• Case Management in an integrated model preferred.
• Minimum 3 years experience case management for Medical necessity/appropriateness

Schedule:

M-F 8 to 5pm

Contract to start

Job Types: Full-time, Contract

Pay: $36.00 - $38.00 per hour
