The Outpatient Pediatric CCS Coordiantor is responsible for coordinating non-clinical tasks to support the delivery of cases who meet case management criteria to the Pediatric case manager. Repsonsiblities include but are not limited to assisting the case manager in scheduling medical appointments, durable medical equipment, home health, eligibility/ benefit verification, requesting medical records, data entry, and review of medical management reports. Also, the outpatient coordinator is accountable for reaching out to memebers telephonically to introduce and invite them to Case Management. The coordinator will maintain high quality and reliable information related to reports, data, cases, and be able to identify and report any discrepancies.

Essential Duties and Responsibilities include the following:

  • Establishes and maintains a relationship with the member and the family where the CM prioritizes and addresses the medical, social, functional, and psychosocial needs of the member.
  • Demonstrate knowledge and understanding of the CCS system to make the appropriate referrals and assist with community resources for this population in order to address the needs of the member. Under the direction of the Pediatric CCS case manager.
  • Ability to identify CCS covered benefits and direct members to utilize appropriate provider and receive timely care. Coordinate with the Pediatric CM member/family/caregiver teaching, referrals and follow-up care for CCS members and work with the team to assist CCS eligible members to obtain coverage as needed.
  • Assists with documents; member assessment and reassessment, individualized care plans, and other pertinent information completed in the member’s medical record

6. Communicates the Care Coordination to physicians and other members of the team to assist the peds CM

  • Demonstrates the ability to follow through with requests, sharing of critical information, and getting back to individuals in a timely manner.
  • Responsible for the coordination of post-discharge clinic appointments, medication reconciliation, PCP and SPC visits, transportation and communication assistance.
  • Collaborates and communicates with all members of the healthcare team.
