POSITION SUMMARY
Contributes to the organization by assigning the appropriate diagnosis and procedure codes, sequencing, and assigning from the CPT, ICD-10-CM, and HCPCS Level II coding applications. Applies all appropriate coding guidelines and criteria for code selection. This position is remote but applicant must be Houston-based.

ESSENTIAL DUTIES & RESPONSIBILITIES
Review patient medical records and abstracts medical data that identifies all diagnoses and procedures. Codes diagnoses, procedures, and appropriate modifiers from the medical record documentation using CPT, ICD-10-CM, and HCPCS Level II classification systems.
Codes diagnoses and procedures from the medical record using CPT, ICD-10-CM, and HCPCS Level II classification systems.
Provide second-level review of billing performances to ensure compliance with legal and procedural policies and to ensure optimal reimbursements while adhering to regulations prohibiting unbundling and other questionable practices.
Audit medical record documentation to identify under-coded and up-coded services; prepare reports of findings and meet with providers to provide education and training on accurate coding practices and compliance issues.
Interact with physicians and clinic staff regarding billing and documentation policies, procedures, and regulations; obtain clarification of conflicting, ambiguous, or non-specific documentation.
Submit any issues or trends found within documentation by a physician and/or clinic to leadership.
Interact with clinic staff to ensure appropriate and complete follow-up of patient accounts to maximize reimbursement.
Research, analyze, and respond to inquiries regarding compliance, inappropriate coding, denials, and billable services.
Provide training, guidance, and oversight to staff less experienced in coding guidelines.
Serve as an information resource and guide to clinicians, champion the need to change coding behaviors, and serve as subject matter expert.
Interact with Physicians and managers to review and/or implement codes and to update charge documents.
Interacts with EMR Team to ensure codes and diagnoses are accurate.
Illustrate excellent knowledge of healthcare industry regarding the revenue cycle, coding, claims, and state insurance laws.
Ensure strict confidentiality of financial and medical records.
Maintains productivity standards set forth in Departmental Policies and procedures.
Maintains knowledge of coding and professional skills, including maintaining yearly coding credentials through attendance at in-service programs, conferences, workshops, review of current literature and other educational programs.
Review and respond to coding questions.
Ensure billed service is being accurately coded.
Perform random chart audits.
Provide continual coding updates.
Research coding issues that arise.
Reviews charts for documentation and signature.
Performs other duties as needed.

COMPETENCIES
EDUCATION High School Diploma or GED equivalent
CERTIFICATIONS & LICENSES Certified Professional Coder/Certified Ophthalmic Professional Coder (CPC, COPC) required.
Ophthalmic Coding Specialist (OCS) is preferred. CPC or COPC candidates will be provided 1 year of employment to obtain the OCS certification.
EXPERIENCE One to two years of experience in outpatient coding and/or reimbursement activities is required.
In addition, a clinic setting background and previous chart abstraction experience is preferred.
