Assurance Health System is a short-term inpatient geri-psychiatric hospital that focuses on the mental health needs of our senior population. Our dedicated staff provides highly specialized and compassionate psychiatric care for our aging population. Our fast-growing health system provides cutting edge treatment for an expanding patient population, with a focus on the quality of care that we provide. Presently, Assurance Health is seeking a Registered Nurse for admissions into one of our facilities.
This individual acts as a liaison between Assurance Health and other long-term care providers to ensure continuity and quality of care for patient admissions.
The successful candidate assigned to this role will function under the supervision and direction of the admissions manager and the director of patient relations. The liaison will plan, direct, and evaluate potential patients through a referral system in accordance with the established policies and requisite level of care. This individual is responsible for assessing the needs of patients to determine admission eligibility, and initiates, implements, and evaluates individual patients for acute criteria with the medical director for patient admission. This position also identifies candidates who meet the program criteria and will benefit from the psychiatric services provided by Assurance Health.
The most important duty of this position is to develop positive relationship with area skilled nursing facilities, assisted living facilities, physicians, physician office staff members, and area hospitals. Liaisons are assigned to specific geographical territories, so applicants must know that this is a travel position with specific territories before applying.

SHIFTS/SCHEDULE:
This job calls for a non-traditional work schedule and requires a dedicated staff member.
EDUCATION/WORK EXPERIENCE:
- BSN or Licensed Social Worker
- Current and Unrestricted BSN License in state of Indiana or Licensed Social Worker
- Experience in Geriatrics preferred
