The Tailored care management program provides community-based care to individuals with severe and persistent mental illness who may be dually diagnosed with IDD. The primary responsibilities of the Behavioral Health Care Manager are to promote whole person, integrated care by planning, coordinating, tracking, addressing gaps in care, and monitoring care, including medication management to meet established measures.
This position is open to candidates living in Wake County, preferably Raleigh, who will be able to perform local, in-person visits as well as virtual visits. Services are delivered in a combination of remote interactions as well as regular face to face community visits. Ability to work flexible hours, including evenings, based on case load availability is required. Crisis phone coverage required (stipend provided) and a FHR offers competitive compensation for this comprehensive position
Bachelor’s degree in a human service related discipline and two or more years of experience working with this population or a masters degree and one year of experience working with this population. Computer literacy required, prior experience with an electronic medical record desired
